Physical Therapist- Home Health

Position Summary: Evaluates and treats patients referred by licensed physicians primarily in the Home Health setting. Completes OASIS admission, recertification and discharge assessments as assigned by Home Health/Hospice Clinical Supervisor. Performs an evaluation, determines a treatment plan, implements treatment plan and modifies as indicated to reach established goals. Responsible for knowledge and performance of Agency, state, and Federal guidelines regarding Home Health care delivery. Provides all patient care services in accordance with North Carolina law, facility/agency policies, and directives of the Medical Staff.
Requirements: Licensure: Full, unrestricted Drivers License; Active License of Physical Therapist in the State of North Carolina. Education: Graduate of a school of Physical therapy approved by the American Physical Therapy Association. Experience: One year experience as a Physical Therapist. Other: Must have an automobile for travel, and maintain current auto liability insurance and have other means of transportation

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
